All students in the Denison High School Touch of Gold Marching must fill out the following forms via RankOne: UIL Cardiac Awareness Form, UIL Steroid Form, UIL Concussion Form, Denison ISD Drug Form, Medical History Form and Denison ISD Social Media Agreement. If you are an incoming freshmen or junior, you are also required to get a pre-participation physical. This can be turned in via email or in person to Mrs. Lewis. All Medical History and Physical forms must be completed prior to summer band.
